2012 RPP Information:
The 2012 RPP Questionnaires have been mailed out to the home addresses on file for those permanent and limited term personnel who are required to submit this information annually. If you received one, please complete it and mail it by December 1, 2011, even if you have completed one in the last few months. Do not assume it is a mistake. Please call the Personnel office if you have any questions.

What you will receive:

1. RPP Letter
2. OSHPros Self-Addressed Envelope
3. CAL FIRE 600a - RESPIRATORY PROTECTION PLAN MEDICAL QUESTIONNAIRE
(Your contact information will already be entered on the form.
Add any missing information that should be filled in.
If any of it is incorrect, please notify your unit Personnel Office.)

4. STD 610 - HEALTH QUESTIONNAIRE
(For Limited Term or Retired Annuitants only)

Once you receive this package, please note the following reminders:

  1. Please read the enclosed letter and follow directions thoroughly.
  2. All employees will complete the CAL FIRE 600a questionnaire and sign and date page 3.
  3. If you are a limited term employee or retired annuitant, you will also complete the STD 610.
    Sign and date in the section marked "Applicant's Signature" on the 2nd page.
    DO NOT sign where it says "Reviewing Authority's Signature".     Do not fill out the section designated "Doctor".
  4. Make a copy of all completed forms for your files, and then place in the pre-addressed envelope provided.
  5. Sign the back of the enclosed, pre-addressed envelope and mail by12/01/2011.
  6. Log into Target Safety to confirm that you have mailed the questionnaire.
